Below are links to the forms that you will need to complete before your first session. I ask that you please print them all out, read and complete them all in their entirety so that I may have all of the information needed to effectively assist you. Please note that if you intend to utilize your health insurance for reimbursement purposes, there are forms below that are specific to this need and they must be submitted back to my office at least two days before your first scheduled appointment.
